Two documents in the parent/guardian or students name tied to the home.
Two of the following Proof of Address from the list below:
- Property tax payment receipts
- Rental property contract, lease, or payment receipts
- Utility service contract, lease, or payment receipts (gas, electric, water, etc.) (Current)
- Pay stubs (Current)
- Voter registration
- Correspondence from a government agency (Current)
- Declaration of residency executed by the parent/guardian
(As stated in E.C. 48204.1 and NUSD AR 5111.1)

What is a Declaration of Residency?
Families who live in a residence other than their home residence (i.e. living with another member of the family or living with a friend). In order to complete a Declaration of Residency, the owner/leasee must be present at the time of the appointment. In addition, the owner/leasee must show two proofs of residency (e.g. current rental agreement/mortgage/property tax, AND a recent utility bill, pay stub, voter registration, or correspondence from a government agency). The parent/guardian and owner/leasee must also present photo identification (e.g. government issued identification).
