Registration Guidelines

2024/2025 School Year

• Children turning 5 years old on or before September 1, 2024 are eligible for kindergarten for the 2024-2025 school year.
• Children turning five between September 2, 2024 and June 2, 2025 are eligible for transitional kindergarten for the 2024-2025 school year.

2023/2024 School Year

• Children turning 5 years old on or before September 1, 2023 are eligible for kindergarten for the 2023-2024 school year.
• Children turning five between September 2, 2023 and April 2, 2024 are eligible for transitional kindergarten for the 2023-2024 school year.

 

2024-2025 School Year

A physical examination from a doctor performed after February 1, 2024. This must include a complete health history, complete physical exam including TB assessment.

 

2023-2024 School Year

A physical examination from a doctor performed after February 1, 2023. This must include a complete health history, complete physical exam including TB assessment.

2024-2025 School Year

Submit proof of an oral health assessment performed by a licensed dentist. This exam must be done after February 1, 2024.

2023-2024 School Year

Submit proof of an oral health assessment performed by a licensed dentist. This exam must be done after February 1, 2023.

 

ORAL HEALTH FORM

ONE or more of the following (as stated in E.C. 48204.1 and NUSD AR 5111.1)

• Rental Property Contract, Lease, or Receipt
• Property Tax Payment Receipt
• Utility Service Contract, Statement or Payment Receipt (such as PG&E or Water Bill)
• Pay Stub
• Voter Registration Form
• Correspondence from a Government Agency
• Declaration of Residency Executed by the Student’s Parent/Guardian

Original Certificate of live birth from the county of birth, not a hospital certificate

If the above is not available, the following will suffice (according to NUSD AR 5111)

• A statement by the local registrar or county recorder certifying the date of birth
• A duly attested Baptism Certificate
• Passport

Immunization records that include:

• Polio – 4 doses
• DTaP/DPT – 5 doses (last doses after the age of 4)
• Measles, Mumps and Rubella/MMR – 2 doses (one dose after the age of 1)
• Hepatitis B – 3 doses
• Varicella/Chicken Pox vaccine – 2 doses
